Tacoma Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current or make changes. You will be required to sign your forms once per year and after any form updates.

A copy of your child's physical is still required and lasts 13 months. Requirements include a doctors stamp , signature, date, and the words "cleared without restriction". Doctors forms can be downloaded using the link below.

To be eligible to participate, students will need the following items:

-Valid Sports physical (Tacoma School District honors physicals for 13 months) The focus of my attention when accepting a physical is the date and the words "Cleared without Restriction".

-Middle School Activities Contract and Eligibility Form. This includes an insurance form that the parent must fill out stating the athlete has insurance (Athletes must have health insurance). This is filled out during on-line registration.

-Emergency Medical Card. This is filled out during on-line registration.

-Concussion information form. This is filled out during on-line registration.

-ASB Card purchased. Cost is $15 and is purchased in main office.

-Students must maintain a 2.0 GPA with no failing grades. Grades are checked at least every other week. Students can bring grades up to be eligible.

-Honor Level 1 or 2 to participate. Honor Level 3 means an athlete can practice but not play in a game. Honor Level 4 means athlete cannot practice or play in games until Honor Level improves (Honor Level is affected for 2 weeks from infraction).
